Private Ambulance vs. 112 / SMURD: When to Call Each
Many patients and families face the same question: "Should I call 112 or book a private ambulance?" Getting it wrong can mean either an unnecessary wait while tying up public emergency resources, or a dangerous delay when a real emergency needs immediate attention. This guide explains, clearly and without jargon, how each system works and when each is appropriate.
Critical and important: for any medical emergency, always call 112
If a patient's life is at risk — cardiac arrest, stroke, severe trauma, active seizures, serious breathing difficulties, or any other acute emergency — call 112 immediately. The 112 emergency service and SMURD are free, available around the clock, and specifically designed to handle medical emergencies. Do not hesitate and do not waste precious time in these situations.

What is 112 / SMURD?
112 is Romania's single emergency number, free of charge and available 24/7. Calls are handled by the Unified Dispatch Centre and routed to:
- SMURD (Mobile Emergency, Resuscitation and Extrication Service) — public emergency medical teams integrated into the national system
- SAJ (County Ambulance Service) — public county ambulances
These services are state-funded, cost the patient nothing, and are optimised for acute medical emergencies.
When 112 / SMURD is appropriate
- Cardiorespiratory arrest or immediate risk to life
- Stroke (CVA) — symptoms: sudden numbness, speech difficulties, blurred vision, severe headache
- Severe trauma (road accidents, falls from height)
- Severe breathing difficulties
- Loss of consciousness
- Active seizures
- Intense chest pain (possible myocardial infarction)
- Uncontrolled haemorrhage
- Any situation where the patient cannot be moved without emergency intervention
What is a private ambulance?
A private ambulance is a scheduled medical transport service, licensed by the relevant authorities, for patients who need medical transport but are not in an acute emergency. It is a paid service, requested in advance (or at minimum a few hours ahead), with an explicit booking.
NucleusMED operates under a sanitary authorisation issued by DSP Brașov and an ANMDMR licence, with a fleet of properly equipped Mercedes-Benz vehicles and crews trained for safe, dignified patient transport.
When a private ambulance is appropriate
- Recurring dialysis — regular transport to the dialysis centre several times a week
- Chemotherapy and radiotherapy — scheduled oncology sessions
- Imaging investigations (CT, MRI, ultrasound) for immobilised or reduced-mobility patients
- Hospital discharge — transport from hospital to home or another facility after a completed admission
- Inter-hospital transfer — moving a stable patient between two medical units
- Post-surgery transport — travel after a surgical procedure when the patient cannot use normal transport
- Specialist appointments — planned consultations for patients who cannot travel independently
- Medical repatriation — bringing a patient back from another country or international transport
- Elderly or immobilised patients who cannot use a private car or taxi

Real-world scenarios: which option is right?
Scenario 1: Patient with chronic kidney failure, dialysis Monday, Wednesday, Friday
This is not an emergency — it is a planned, repetitive medical transport. The 112 emergency service is not designed for this type of journey. The right choice: a private ambulance, with a weekly schedule and a crew familiar with the patient's needs.
Scenario 2: Acute chest pain, cold sweats, sudden onset of illness
This could be an ongoing myocardial infarction. Call 112 immediately. Every minute counts.
Scenario 3: Patient discharged after hip replacement surgery, unable to travel independently
The discharge is planned and the patient is medically stable. A private ambulance provides comfortable, door-to-door transport, with a crew to assist with boarding and ensure safe travel.
Scenario 4: Elderly person with limited mobility, specialist cardiology appointment in another town
Not an emergency, but the person cannot use public transport or a taxi. A private ambulance is the right choice — comfort, safety, and a companion if needed.
Why does this distinction matter?
The 112 and SMURD emergency services are limited public resources dedicated to life-threatening situations. Using them for non-urgent transport can block an ambulance that might have saved a life. Conversely, waiting for a private transport booking during a real emergency can be fatal.
Understanding the difference between the two systems is both a civic responsibility and a decision that can be critical in a moment of need.
