What to prepare before discharge
A well-planned discharge means both the family and the medical team can anticipate the patient's needs.
Documents to prepare
- Discharge summary — states the diagnosis, treatment, and physician recommendations
- Referral letter or any recommendation for continued home care or transfer to another facility
- Medication list with prescribed doses and administration schedule

- Floor, ward number, and hospital name for patient pick-up
- Whether the patient needs a stretcher, wheelchair, or can walk with support
- Any attached medical devices (oxygen, IV drip, drain, catheter)
- Exact home address and access conditions (floor, lift availability, number of steps)

Home access conditions
The NucleusMED crew is trained to transport patients through stairwells, narrow lifts, or difficult terrain. Providing this information in advance ensures the right equipment is prepared: a stair stretcher, evacuation chair, or harness lift.
How a NucleusMED hospital-to-home transfer works
- Phone booking at +40 758 409 100 — date, time, and logistics are confirmed
- Confirmation and preparation — the crew reviews the transport record and prepares the ambulance accordingly
- Pick-up at the ward — the team arrives directly at the patient's bedside with a stretcher or wheelchair
- Safe transport — Mercedes-Benz medically equipped vehicle, authorized by the Brașov Public Health Directorate (DSP Brașov), with clearance from ANMDMR (Romanian National Agency for Medicines and Medical Devices)
- Handover at destination — the patient is comfortably settled at home or transferred to the chosen care facility
All stages are carried out by a certified crew, under the sanitary authorization issued by DSP Brașov, in compliance with the requirements of the Romanian Ministry of Health for medical transport services.
Assisted vs. non-assisted transport — what's the difference?
Non-assisted transport is suitable for stable patients who do not require medical monitoring but cannot travel independently. The crew includes a driver and a trained medical orderly.
Assisted transport includes a nurse or, on request, a physician. It is recommended when:
- The patient's condition may change during the journey
- The patient depends on continuous oxygen supply or an IV drip
- The attending physician explicitly recommends monitoring of vital signs

Why can't you call SMURD or 112 for discharge transport?
SMURD ambulances and 112 emergency services are intended for active medical emergencies — situations where the patient's life is in immediate danger. Hospital discharge does not constitute a medical emergency, and requesting an emergency crew for a planned journey blocks resources that other critical cases urgently need.
